Financial Information

Those without dental insurance:  Payment is required at the time of service.

Those with dental insurance:  We work closely with CSEA, Guardian, and Metlife.  We will contact those insurance companies for an estimate of coverage on your behalf. Although generally accurate, we cannot guarantee this will be an exact reflection of your coverage once processed by your insurance.

If your insurance pays more than the estimated amount, a refund check from Queen City Endodontics will be mailed within 1 month from the date payment is received. 

If your insurance pays less than the estimated amount, you will receive a statement from Queen City Endodontics. Prompt payment is greatly appreciated!

If your insurance company does not reimburse after 2 submissions, you will be responsible for the remainder of the balance.

Payment methods accepted include cash, personal checks, Visa, MasterCard, Discover, American Express, and third-party financing through CareCredit.

If payment is made with a check and the check is returned for any reason, a fee of $50.00 will be added to the balance owed.

For any other insurances unfortunately the check will go to the patient. We will file your insurance claim for you, but payment is due in full at the time of service.  We will also be more than happy to submit a pre-treatment estimate on your behalf so you have an idea what to expect from your insurance.

Do you offer a payment plan?

We offer an alternate financial option, which is CareCredit (www.carecredit.com). You will be applying for credit directly with CareCredit. Their approval process is simple and you can apply online before making an appointment with us. Both 6 and 12 month payment options are available through CareCredit.
